Male Factor Contributes To ~40% Of Infertility
Appropriate treatment is based on identification of cause which may be varicocele, damaged testes or genetic causes.
varicocele is a common cause, where the veins around the testis are enlarged damaging the sperm quantity and quality. Microscopic (using operating microscope) varicocelectomy helps in improving sperm counts and quality and a better outcome with infertility treatment. We use intraoperative microvascular Doppler ultrasound to identify the testicular artery to ensure superior outcomes.
Surgical sperm retrieval (testicular/ epididymal and using an operating microscope) is a specialised procedure which allows identification of the occasional precious sperms from inside the testis in an attempt for own gametes (Non obstructive azoospermia)
Surgical correction of blocks in the passage to aid in natural conception (Obstructive azoospermia)
Unconsummated marriage or erectile dysfunction, can itself be a cause for infertility and appropriate treatment is provided for natural conception if rest of the parameters are normal
Men having difficulty in sex and those especially beyond 35 years need to be checked for other problems like diabetes mellitus, heart diseases and testosterone deficiency. Therapy includes oral tablets, intra penile injections, penile prosthesis implantation and counselling
Bending or penis due to Peyronie disease: Evaluation of plaque, medical therapy and injections. Surgery may be necessary in patients with no response to medications
Causes include epididymo orchitis, epididymitis, varicocele, etc. and appropriate treatment is provided using oral medicines for pain relief, surgical correction of cause.
Temporary pain relief is possible with injections and nerve blocks.
Long term pain relief is possible through microsurgical spermatic cord denervation using an operating microscope.
Suspected if you are becoming increasingly tired recently, with decreased energy, interest and reduced sex drive with impotence and hot flushes, gaining weight, irritability and alteration in sleep pattern
Few blood tests including testosterone, and heart check-up will be done as indicated
Lifestyle modifications and testosterone therapy if indicated, to assess response
For those men who are getting married, premarital check-up is done with the help of basic blood tests, clarify doubts regarding erection and provides an avenue to identify unprecedented sperm problems
Men’s health check is an unique check-up which is based on patient symptoms, required tests, physician consultation, dietician consult for tips on healthy diet, physiotherapist consultation for lifestyle changes and exercise advice and andrologist counselling for a holistic approach to men’s health
For those men who are getting treated for cancers with chemotherapy or radiotherapy and operations for certain cancers, sperm cryopreservation is a boon to preserve sperms for future paternity. Chemotherapy and radiotherapy can affect sperm production
Men with progressive low sperm counts can also discuss the option of cryopreservation while on treatment for improving sperm counts
